Nurse's Office - Medical Forms and Medication
Complete all medical forms and bring medication as soon as possible. All medication must be brought in by an adult and given directly to the nurse—students cannot bring medication to school. Forms expire at the end of the school year and must be completed and updated yearly. Forms were sent home at the end of last year and can be found on the Lumberton website under Family Tab > Forms for Families > Medical. If your student will be absent, call the attendance line or email the reason and date. To be excused, a doctor's note is required. Students must be fever-free (without fever-reducing medicine) and vomit/diarrhea-free for 24 hours before returning (fever is 100.4°F and above). The nurse will be available at Back to School Night to accept forms and medication. Notify the nurse of any health or medication changes during the year.
